Referral Details

Based on past experience we decided to have an open pathway which often means we receive referrals from the following:

  • Self-referral from young people or family
  • GP or other doctor involved with the child
  • Any Health professional involved with the young person
  • SENCO
  • Children with Disabilities Social Work Team

Referrals can be received over the phone, via email or in the post they are then screened to ensure they meet our criteria.

Our hours of working are Mon-Fri 8:30am – 4:30pm (we can occasionally work outside of these hours)

We do not currently have waiting times in place, although this is subject to change dependent on capacity.

Available to those on our caseload, we have the facilities for young people to call, text, email or facebook.

Local Offer

Description

The Health Transition Team offer person centred support and advice to young people who have a physical impairment and are preparing for adulthood. We complete a comprehensive assessment to determine transition readiness and provide advice and signposting to other services as appropriate.

We offer this to young people from Walsall from Year Nine onwards who are in mainstream education. Young People (14- 25 years) who have long term physical health condition, such as Cerebral Palsy, Duchenne Muscular Dystrophy for example.

Support provided includes:

•Holistic support, helping young people to gain access to services available within health, education, social care and leisure.
•The development of a Health Passport or one page profile
•1:1 Case Management, peer or group support in a range of appropriate and accessible settings.
•Promoting self-efficacy and self-care management (becoming confident in self and own abilities).
